Applicant Privacy Notice
1. What this notice covers
This notice explains what we do with the information you give us when you apply for a job with Wellington Internal Medicine Group. It applies to applications submitted through the careers page on our website, sent to us by email, handed to our staff in person, or passed to us by a recruiter.
This notice is about you as a job applicant. It is separate from our Notice of Privacy Practices, which covers you as a patient. If you are both a patient and an applicant, the two are handled separately and by different people.
2. What we collect
From the online application form:
- Your first and last name
- Your email address and phone number
- The position you are applying for
- The location you would prefer to work at
- Your years of experience
- The earliest date you could start
- Your resume, and whatever it contains
- Anything you choose to write in the open message field
We may also collect later in the hiring process:
- Professional licenses, certifications, and their verification
- Education and employment history, and the results of confirming it
- References you give us, and what those references tell us
- We run a background check and E-Verify
- Notes our staff and physicians write during interviews
We do not ask for your Social Security number, date of birth, government ID numbers, or bank details on the online application form. We only ask for those after we make you an offer and you accept it.
We do not ask about your race, color, religion, sex, national origin, age, disability, veteran status, marital status, pregnancy, or genetic information as part of evaluating your application.
3. Please leave medical information out
Our application form is not a place to send medical information. Do not include your own health details, and do not include any information about patients you have cared for at another practice. If you send us medical information anyway, we will handle it with the same care as everything else in your application, but we will not use it to evaluate you and we may delete it.
4. What we do with it
We do not sell your information. We do not use it for marketing.
5. HIPAA and your application
Your job application is not treated as protected health information under HIPAA. HIPAA covers information about patients.
6. Who else sees it
Inside the practice, your application is seen by the people involved in filling the role: our office manager, the practice manager at the location you applied to, and the physician you would work for.
Outside the practice, your information reaches:
- Our website and hosting providers, who store the form submission on our behalf
- Our email provider, which carries the notification and any correspondence with you
- Government agencies and courts, when the law requires us to produce it
Each of these providers is given only what they need, and only to do work for us.
7. How long do we keep it
If we hire you, your application becomes part of your employment record and is kept for as long as that record is kept.
If we do not hire you, we keep your application for 4 years so we can consider you for other openings. After that we delete it or destroy it.
9. Your choices
You can:
- Ask what we hold. We will tell you what is in your application file.
- Correct it. If something you gave us is wrong, tell us and we will fix it.
- You can ask us to stop considering you and delete your application at any time.
- Decline to answer. Every field beyond name, contact details, position, and resume is there to help us place you well. You can leave the rest blank and still apply, though it may slow us down.

Withdrawing does not affect any decision we have already made, and we may keep a minimal record that you applied for and withdrew if a recordkeeping rule requires it.
11. Changes to this notice
If we change how we handle applications, we will update this notice and change the date at the top. The version on our website is always the current one.
